Cannot be defragment

When I defragment my hard drive there remain some files that cannot be defragmented. Specifically 126 fragments this time around. File size is 108 MB.

The files that cannot be defragmented are listed as:

\Documents and Settings\ my name

based on the information supplied can you tell me why some files cannot be defragmented?

Usually when this happens is when there are blocks on your hard drive most experts will tell you once you start seeing something like the it is recomended that you replace the hard drive ASAP before it spreads The best thing i can suggest is start backing up your important files and start working on installing a new hard drive but to be sure do anothere disk defragment just to be sure ok.
