How do I make an XP boot disk?

I need to make a boot disk for winxp. How do I do this?

With it, I am going to reformat my hard drive. What commands should I use to reformat, and delete/create a new partition for my c drive?

You don't have too, all the boot ifomation is on the Win XP os CD You will be able to create your partitions from the cd when you insert in the CD Rom drive and it starts the set up.
